LCC Kids Registration 2025-2026

Each year we ask parents to register their children* for LCC Kids. Registering ensures we have up to date contact information and gives us an estimated head count for our classes. Only infants through 5th grade students need to register. If you have any questions, please reach out to Pastor Gordon, Kalika Ayin or Debbie Ong.
Please submit one form per child. This ensures that all contact information is up to date on Breeze, and allows the LCC Kids team to plan accurately for the school year. Mahalo!

Photo/Video and Medical Release
LCC Kids and Leeward Community church are committed to providing a safe and nurturing environment for your child. This section grants permission for photo/video use and medical care in case of emergency, and outlines liability of release for participation in church-sponsored activities. 

Photo/Audio/Video Release
I am the parent or legal guardian of the above-named child. I hereby grant Leeward Community Church its representatives, employees, and volunteers permission to take photographs and video recordings of my child(ren) during church events and activities. 

I understand and agree that: 

These images may be used internally (e.g., newsletters, ministry reports, slideshow presentations)
These images may be used externally (e.g., promotional materials, church website, social media platforms). 
My child’s full name will not be shared publicly without additional written consent. 
No compensation will be provided for the use of these images. 

Medical Release and Liability Waiver
In the event of illness or injury occurring to my child while participating in any activity sponsored by Leeward Community Church, I authorize the adult leaders, staff, or designated volunteers to secure necessary medical treatment for my child, including transportation to a medical facility if needed. I understand that reasonable attempts will be made to contact me before such action is taken. 

I agree to be responsible for any medical expenses incurred. I also release and hold harmless Leeward Community Church, its pastors, leaders, employees, volunteers, and agents from any and all liability, claims, demands, damages, or costs arising from or related to any injury or illness that may occur while my child is participating in any church-related event or activity, except in the case of gross negligence or intentional misconduct.
